Do I have to be referred by a doctor?
Yes, Texas requires patients to have a physical therapy referral from their PCP or other licensed healthcare provider. You can either bring the referral with you for your first appointment or have your healthcare provider fax it to us. Our fax number is (817) 786-8057.
What do I need to bring to my first appointment?
The day of your first appointment please arrive 15 minutes early to fill out paperwork. Be prepared to provide your previous medical or surgical history. Please bring a government issued ID (like a driver's license) and insurance card(s).
If your healthcare provider has not already faxed us your referral, then please make sure to bring your referral with you as well. You may wish to bring comfortable clothing.

If you decide to fill out your new patient paperwork ahead of time, then please arrive a few minutes early, so that our front desk can make copies of your ID and insurance card(s).
After a thorough 45 minute to 1 hour initial assessment, the physical therapist will discuss your treatment. However, be aware treatment duration depends on your diagnosis, the severity of your impairments, your past medical history, and other factors. Re-evaluations will be done to assess your progress and your therapist will communicate regularly with your physician to help you achieve your therapy goals.

We will check with your health insurance provider to determine if outpatient physical therapy services are covered under your plan. We recommend that you confirm your insurance coverage.

Please call us at (817) 786-8058 to cancel or reschedule your appointment. Please be aware we have a 24 hour cancellation policy. A cancellation fee of $40.00 will be charged for each appointment that is cancelled with less than 24 hour notice. The fee is waived when you reschedule your appointment in the same calendar week.

1) Yourself- you limit your ability to reach your goals
2) Your Therapist- time has been made in the therapists schedule specifically for you
3) Another Patient- We are unable to fill your appointment slot with others that are needing to get on the schedule when short notice is given
We are happy to work with your schedule and the fee is waived when you reschedule your appointment in the same calendar week.
